Request an Appointment
Patients can request a Non-Emergency* appointment at any time. Please feel free to request an appointment by calling our office directly, or by filling out the form at the bottom of this page and submitting it to us.
Please understand we will make every effort to accommodate your request, but due to previously scheduled appointments, we may need to offer an alternative date or time. Please note that your requested appointment is not a confirmed appointment until someone from our office responds to your request (usually within one business day).
Finance Information
Insurance:
We are providers for Medicare and certain plans for: Blue Cross/Blue Shield of Florida, United Healthcare, Tri-Care, Railroad Medicare, Humana Gold, Great West, Cigna, Coventry, Beech Street, Aetna, and others (updated 3/1/10). Please verify your specific plan with our billing department. If you have another insurance plan, we will file your claim for you as a courtesy; payment is expected on the day of your exam or procedure.

We accept VISA, MASTERCARD, and DISCOVER CARD.
